Before you come for your first appointment, we thought you might appreciate knowing a little bit about us and getting an idea of what you can expect.

Before Your First Appointment

Before your first visit, we'll ask you to fill out a medical history form, which is vital in developing your treatment plan. You can find additional forms we require here on our Forms page. You will receive a welcome letter from our office that includes a reminder with the date and time of your appointment.  

The night before your appointment, our dentist like to call new patients to introduce themselves. They believe it is helpful when establishing rapport with patients. They will ask if you have any specific concerns or questions.   

Please plan to bring the following to your first appointment:

  • Personal Identification, (a driver's license or passport works well)   
  • New patient forms, if you have completed them in advance
  • Current medication listing
  • Dental insurance card or information
  • Any questions you may have about our office or your dental health care

Step Inside for Your Office Tour

When you arrive, you will be welcomed by our patient greeter. She will introduce herself and gather and process your patient forms.

Next, she will begin your patient experience by giving you a tour of the office. You will notice our comfortable and friendly reception area, and she will show you where the restrooms and treatment rooms are located.

You will then be seated in our consultation room and given an opportunity to finish any needed paperwork and open your new patient gift. We would like you to be aware of the comfort menu available in our office and have it printed for your review.

Meeting Our Team

We will introduce you to one of our dental assistants, Denise or Sadi, who will escort you to your treatment room. They will take any necessary x-rays for your exam and ask you a few questions to learn more about your values and concerns relating to dental care.    

Next, the assistant will introduce you to Dr. Waters or Dr. Crouch, and he will spend the next few minutes getting to know you. He will then examine your teeth, gums, jaw alignment, and soft tissues. He will review your x-rays, check for oral cancer and examine your bite.  

Developing a Treatment Plan

Later, using our findings from the exam, we will determine a treatment plan and go over it together. You will be introduced to Cyndi or Echo, our patient services coordinators who will assist you during this portion of your appointment. They will meet with you to go over your treatment plan, make financial arrangements, and, if you have dental insurance, explain insurance estimates. We ask that you pay your estimated portion on the day treatment is complete.

If you do not have insurance, we ask that you pay for services in full on that day. We will schedule an appointment for a dental cleaning with one of our hygienists in addition to any other restorative appointments you may need.
